A new website has been designed to help people better manage chronic conditions such as diabetes and heart disease.

MyVirtualHealthCheck.com offers users with a suite of advanced Web-based tools that provide a method to record, monitor, and share important vital sign data such as blood glucose levels, blood pressure (BP), heart rate, and weight with their doctors, caregivers, and loved ones. More...
The process is safe, private, and convenient, using a secure Web interface, mobile cellular devices, a PC gateway compatible with Bluetooth wireless transmission, and in-home monitoring devices. A host of Web tools is offered, including a food diary, a physical activity log, mobile applications, and a nutrition information database of more than 14,000 foods, including fast-food menu choices. MyVirtualHealthCheck.com was developed by Chronic Care Management (San Antonio, TX, USA).
